Miles Ahead: Order Form

Thanks for your interest in the Miles Ahead Items for Sale List. Use this form to assemble an order after you have consulted the Miles Davis, Charlie Parker, Other Jazz Items, and Non-Jazz Items sale lists.

Here's how the process will work:

1. Be sure to include your full mailing address and e-mail address. Make sure they are correct.

2. Copy the full title, and the price, of each item you want from the sale list into the "Item(s) Wanted" section of the form.

3. After you submit your order, I will reply via e-mail with information about the availability of items and total cost including shipping. At that time I will mark the items as sold, which will remove them from the sale list.

4. Please pay promptly -- say within 48 hours (PayPal) to 72 hours (cash or money order). If I don't receive full payment in a timely fashion, I will return your items to the sale list for others.

5. I will ship your items as soon as I can after receiving your payment.
